Intake Forms Instructions

  1. Open the email from SunPath Psychotherapy Client Portal (yourprovider@simplepractice.com).

  2. Click the link to access the Client Portal

  3. Consent to the Privacy Policies for our Electronic Health Record System (SimplePractice)

  4. Fill out each form (you can save your progress and return later)

  5. Submit your forms before your first session

Demographics

  • Please note this form cannot be customized and currently uses binary gender fields due to insurance billing requirements. This is not ideal and we apologize. If you cannot select an option that matches your identity, please choose the closest option and let your clinician know—we will note it in your record.

  • You can use this form to set your appointment reminder preferences.

Credit Card Authorization & Information

  • You will be prompted to enter your card details here. A credit card is required to keep on file. It will be used for missed session fees and for session payments if not using insurance.

Notice of Privacy Practices

  • This form is required by law and explains how your health information may be used and disclosed, and how you can access it.

SunPath Psychotherapy Intake Questionnaire

  • This form helps your clinician understand your experiences so we can create a treatment plan that fits you.

  • The first 17 questions are required (about 5 minutes to complete).

  • The rest are optional, but the more you share, the more tailored our approach can be.

Release of Information

  • Optional form that allows your clinician to collaborate with other care providers (past or present).

In Case of Emergency — Telehealth

  • This form provides SunPath's emergency policies and collects your emergency contact information.

Informed Consent for Psychotherapy

  • This form details policies on fees, insurance, cancellations, attendance, communication, telehealth, and confidentiality.

  • Please read fully before signing.

Appointment Reminders

  • Email reminders are sent 72 hours before your session, and again 10 minutes before, including the session link.

  • You can opt-in for text reminders via the Client Portal.

Billing & Fees

  • SunPath is out-of-network for insurance but can provide documentation for reimbursement.

  • Payments are processed through Stripe via SimplePractice.

  • Leah: Individuals $175, Relational $175; Lirone: Individuals $150, Relational $160

How to Access the Client Portal

Once you’ve completed your intake forms, you can navigate to the Client Portal to:

  • Find upcoming appointments

  • Complete or review your forms

  • Access billing information

  • Send secure messages to your clinician

  • Upload documents
